RELEASE & WAIVER

While every effort is made to ensure your safety while you train, this is physical activity and

although rare, intense fitness training carries a risk of damage to property, personal injury or

death. You must exercise your own judgment as to the difficulty of training, and your ability

to safely protect yourself from inherent risks and dangers. The Circuit Factory wishes its

participants to exercise and train safely within the boundaries of their own capabilities.

 

THE FOLLOWING IS A RELEASE OF CLAIMS AND WAIVER. PLEASE READ BEFORE SIGNING BELOW.

By affixing my electronic signature to this document, I attest, contract, understand and agree that I am to be legally bound by its contents.

I HEREBY STIPULATE AND AGREE:

​

• I willingly participate in The Circuit Factory exercise solely at my own risk;

​

• I, my heirs, executors and administrators hereby waive any claim, right of claim or cause of

action which may arise out of loss of life, injury, damage or loss of any kind;

​

• I attest that I am physically fit and medically able to participate, and (except as previously

expressly disclosed to The Circuit Factory) have no physical restrictions, disabilities or any

pre-dispositions to sickness or injury that may be aggravated or adversely affected as a

result of taking part in The Circuit Factory exercise;

​

• I assume all risks, and take full responsibility, for any injury, loss or damage associated with

or that may arise directly or indirectly from such training, including but not limited to falls,

contact with other participants, effects of weather, traffic and conditions of the road and/or

gym, all such risks being known and appreciated by me;

​

• I hereby waive all claims against The Circuit Factory, its instructors and/or employees,

individually or otherwise, for any and all claims for injuries or damages I sustain, and I

expressly agree not to penalize, prosecute or claim compensation from The Circuit Factory or

any of its instructors and/or employees in the event of any injury, loss or damage.

 

Additionally, I give free permission for the free use of my name, voice or picture in any broadcast, telecast or advertising promotion created or published by The Circuit Factory.

I have carefully read and understood all the information above. I understand that I will

only be permitted to participate in The Circuit Factory by agreeing to and acknowledging all

of the information above.
